Care Manager – These are health care professionals, typically a nurse or social worker, who arrange, monitor or coordinate long-term care services. Also referred to as a care coordinator or case manager. Care Managers not only work for health plans and hospitals, but are often contracted directly by families to assist in the development of care plans. A popular and growing field is geriatric care management. These care managers assess an elders needs and put together a specialized care plan for the individual. Very often they stay on to coordinate the implementation of the plan, the selection of providers and act as an ongoing advocate for the elder.
