Program Description for Providers

What is the ElderCarelink Program?
ElderCarelink is an internet-based referral service -- free to consumers -- that specializes in eldercare case matching for elders and their families. ElderCarelink receives thousands of new inquiries each month from consumers across the United States searching for eldercare products and services. ElderCarelink assists families in finding a multitude of services, including: assisted living, nursing homes, adult day care, homecare, nursing, care management, and in-home products and services. Our program captures information from people in search of private-pay as well as Medicare and Medicaid funded solutions. ElderCarelink’s costs are covered by the participating providers, who pay a set-fee for each inquiry (or lead) that matches their target customer profile.

How Does the Program Work?
We provide a service to eldercare consumers that assists them in finding eldercare providers and/or companies that meet their needs. In essence, they provide us with information about their individual situation, what they need (or think they need), and we do the legwork.

Consumers fill out an on-line profile (“needs survey”) of their needs and preferences and authorize us to match them up with providers and businesses in our network that meet these needs. The profile gathers a combination of general demographic information (e.g., age, gender, location, etc.) as well as information about medical condition, financial budget, payment preferences, insurance, desired services etc.

The Matching Process
Once a consumer submits the necessary information, we compare their needs and preferences with our network participants’ capabilities and preferences and apply a match. The consumer is then notified (via e-mail) of the companies and providers in our network that meet their needs – they also receive contact information and a URL link to the service/product provider’s website or targeted webpage. Consumers are notified that these providers will be contacting them or if they have an urgent need they can contact the provider directly through the contact information we provide them. At the same time, we provide our Participating Providers who matched up with the consumer’s needs and preferences with the contact information and the results from the consumer’s needs survey so that they can immediately follow up with the consumer.

Pricing Based on Selection Criteria
Participating Providers pay a “per lead” fee for the leads that meet their qualifications – they only pay for the leads that match the selection criteria that they defined. Participating Providers can determine how limited or broad they would like the customer qualification screening to be. However, we must ensure that the product or service is available in their area and that it meets at least one or more of the customer’s expressed needs or preferences.

The price per lead that a Participating Provider pays to ElderCarelink is determined from a standard fee schedule that reflects ElderCarelink’s cost of qualifying customers. A base price per lead is calculated based on minimum screening requirements for each provider. Additional screens can be added for an additional fee per lead.

Provider Participation Standards

Providers who would like to become a member of the ElderCarelink provider network and participate in its associated referral service program must meet the following eligibility requirements:

  • Complete and submit the ElderCarelink provider applications;
  • Provide evidence of proper licensure and/or certification required to conduct business and to provide the services for which they are seeking to receive referrals;
  • Provide evidence of General Liability and Medical Malpractice Insurance, if applicable;
  • Provide names and contact information of references if required by ElderCarelink;
  • Authorize ElderCarelink to conduct background checks and, if a Medicare provider, authorization to allow ElderCarelink to verify that the Provider has never been sanctioned by Medicare or any other Federal or State health care program;
  • Accept the terms within the ElderCarelink Participating Provider Agreement.

ElderCarelink reserves the right to deny participation for any Provider who: (a) fails to provide ElderCarelink with satisfactory evidence of any of the required information referenced above, or (b) is discovered through the reference and/or background checks referenced above to have a record that would - in the sole judgment of ElderCarelink -- place ElderCarelink at risk by referring consumers to said provider.
